So speaking of all the end of life thoughts lately, a friend of mine needs advice on seeking out elder care (in IL) and resources for funding it. Her dad has early onset dementia (at 60) and it's severely affecting her mom's life as his caregiver, as well as him of course. They would love to get him into a home where he can be more stimulated as well as free up her mom's life.

This is the correct answer. Especially if you're fasted and want to stay that way, but realistically there's nothing wrong with a diet red bull or sugar free whatever all things remaining equal. The acidity not great for the chompers but artificial sweeteners are one of the most studied compounds in the world and the consensus pretty much seems to be, yeah this stuff can cause cancer and kill you but it's impossible to consume an amount that would from energy drinks.

There is also a formula for how much caffeine you can consume and not be consuming too much based on body weight, so maybe look into that if you're a jittery anxious mess after 3 or something.



Today has been a bit of a slog, first time sitting down since 7am. Hit the grocery store, mowed, whipper snipped, cleaned bathrooms, did laundry including folding and putting it away, vacuumed, mopped, packed an over night bad, went back to the grocery store to get the four or five things I forgot at the first visit. Cooked beef in ale and stock with onions and garlic for a savory pie filling, made thousand layer potatoes prepped to fry, made bischoff fudge, currently infusing honey to make enough hot honey to give as gifts for 6 people and wrapping presents.

First cold beer of the day was like an angel pissing on my tongue.

I have extremely low expectations because Disney flat out loving sucks at everything they put their hands on at the moment and wonder woman 2 was so bad I turned it off 20 minutes in, but I'll give it a shot eventually.

so one thing I found out is there's like, brokers. There's elder care experts you can call who can help with placement, they know all the options in the area and roughly what they cost. We found one via a friend of a friend but they have listings too. There's a huge range of care options, from "day care" style places to small group homes to larger institutions, and a huge range of levels of care including the minimal (independent senior living), more (assisted living), more (nursing home), more (acute care), and specialty (e.g. memory care).

Start with the state site, it'll have an overview and listings: https://ilaging.illinois.gov/programs.html. Check medical insurance, if there is any, for what they cover. Review medicare coverage. Work with primary care physician to get them to prescribe things: not just meds but PT, OT, etc.

You also want to get familiar with your ombudsman office. Why? Because, and I hate to say this to someone in your position, but: survival rates in long-term care facilities are basically always worse than home care. By miles. Even the really expensive ones. But especially the vaguely affordable ones. Elder care facilities never have enough staff to have someone supervising a person 24/7, and as memory care gets worse, that's what a person actually needs: a constant companion. Worse, memory care facilities are full of memory care patients, and they talk crazy and do crazy stuff and make bad noises and your family member is exposed to that constantly, which makes their own mental state much worse. They have to cook food for everyone and serve it to everyone so it's institutional food. They have to deliver meds to everyone so while you can have a schedule like "four times a day" for a pill and that will happen, you can't have a schedule like "give the Ritary at 6:15, with no food, so that she can sit down to eat at 9 and have at least 30 minutes" because protein reduces the effectiveness of the pill but the pills' effects are needed for the patient to manage to eat so if they're not done by 9:30 they start to crash but can't have their next dose till 10:30.

The ombudsman's job is to assist you in making drat sure facilities deliver what they're supposed to. You want to have their number so you can request review, involve them if any issue is contentious, file complaints, etc.

An intermediate option that is far better for patient outcomes compared to institutional care is in-home care. This is of course expensive too, but patients with low budgets can get some of it for free via medicare - medicare and many insurance companies have finally figured out that in-home care is far, far less expensive for them than a facility. I forget the number right now but medicare max in-home care is something like 30 hours a week? Which is not enough, of course, but can be a huge assist to a caretaker who needs someone to help with things like bathing or cooking.

If you have to pay for in-home care out of pocket expect it to be at least $30/hr with a minimum shift of 4 hours, something like that. That's via an agency. You can also direct hire, via care.com for example, and that's potentially cheaper but now you're an employer with all that entails: you have to set up paychecks, withhold taxes, the whole shebang. Direct-hire also doesn't give you the agency's work pool, e.g. if your worker is out sick or on vacation an agency can usually have another person fill in. But if you need a lot of hours and the difference between $20 and $35 is a big deal, it's an option.

That said, in addition to general in-home care there's all kinds of in-home specialist visits you can qualify for, like occupational therapy, speech therapy, and physical therapy. Depending on needs. All of this stuff can really help a caretaker manage.

Lastly, check out respite care. That's where you can drop off an elder for some length of time ranging from one day to more than a month, so the primary caregiver can have a break. Anyone doing in-home care long-term needs to do this. Even though it means an institutional or at least group home setting, it's temporary, you can check out the options, and the patient gets to go home afterward.

Every time my mom went into a facility for more than 2 weeks, she lost weight. They always messed up her meds at least once. She was always more confused, more distressed, more delusional. But it was worth it because my stepdad didn't break down and lose it. Regular breaks can be the difference between coping and completely failing.

I mean, you don't really need to talk while food is in your mouth. You can hold up a finger and get that stuff down first.

But I am not a politician who thinks it's their job to talk whether or not they're saying anything worth hearing. Seems to me taking a big bite would have been more or less fine if it wasn't followed up by talking between the chewing.

Lamb chop crown is our traditional Xmas meal, you rub a bunch of garlic, thyme, and coriander all over them, and then stand the crown up in a bundt pan (with the pan's hole sticking up between the two tied together racks). It cooks in a surprisingly short time (45 mins or so for medium), and is one of the highest delicious-to-effort-holiday feasts I've ever done. Roast brussels sprouts, roast parsnips, roast carrots in hot honey while the meat is resting and then I slice the chops individually and warm them back up in a skillet/cook them to desired temp. Slather on some mint sauce and make a mustard & rosemary gravy with the pan drippings and some sherry wine vinegar. Beautiful.

I just don't see a disney project able to have actual character development over the course of a season at this point but I sincerely hope I'm wrong for this series.


I am finally finished taking everything from ovens, stoves and hot water baths and get put me feet up. I'll have some photos if I don't gently caress up the actual cooking and presenting part tomorrow. At least the prep all went perfectly.

1. Needs a diagnosis from a neurologist. Eventually she’s going to write a letter that’s going to require a diagnosis and neurology never gets questioned. Primary cares will get sent back. Then he’s going to need to apply for disability which won’t be hard if a neurologist is behind it. This will also qualify them to get his funds if they’re a caretaker. So that’s like 3600 bucks or so a month. If one of them stops working they get even more but I don’t know the exact numbers on that.

2. Get everything out of his name. That means houses, cars, boats, whatever. Just sell it to his family for a dollar or something. Things he own will eventually be used to figure out much state assistance he qualifies for. Lots of people fall into the “donut hole.” They own too much to qualify for state benefits but don’t have enough money to pay for things. Estate lawyers can help with this.

3. Have the PCP put together a form called an FL2. This is just a list of all their diagnoses. If it comes time to go to an LTC facility that’s how they kind of shop the patient to places.

4. Avoid places like A Place For Mom. They basically figure out a. assets b. life expectancy and pair them in a facility where both will end at the same time. It’s a gruesome business model.
