There’s a few different ending options around if you let the player character delete Johnny Silverhand from your brain or not. In the ending where you let the player character’s self “die” and let Johnny take your body over, he ends the game by implying he’ll mod the body to his preferences. So there’s an implication he’ll swap out the genitals and voice and stuff but also he might not since it’s one ambiguous sentence. Like even contemplating this is opening up complicated hypotheticals about how you handle giving your whole body over to a distinctly different person as your last wish before death and crap in an alternate future reality where every aspect of your mind and body are as swappable as computer parts.
