I think I'm just biased because I had to deal with a lot of insufferable "All officers are bad grunts know everything the miltiary shouldn't have any non-combat arms" people in formative years. If your fail state of one guy not doing his job is that the loving ship sinks, you are dealing with a systemic issue, not a human error. Why was it not known the doors were not properly closed?
