originally an obscure export control error, it found new life as a Sneakers reference. first used iirc in the copy protection code or boot loader of the first xbox (or maybe in the driver signature code? idk)

not really, since this is basically the equivalent of a kernel panic in the modern windows world. Although I seem to remember that in Windows 9x you would get bluescreens for poo poo like removing a disk or CD that was in use and you could just hit escape to dismiss them

it depends if it was just a DOS program not responding (in which case it might have corrupted memory so it's a good idea to reboot asap) or if it's an actual error windows encountered. Raymond Chen has a blog post about it somewhere.

e: on win95, it meant a device driver crashed. on win 3.1, it meant you pressed ctrl+alt+del to get a special message from steve "sweaty" ballmer

mostly because of hilarious 90s ARM chips

dunno if its been fixed yet, but a couple of years ago at work we discovered that its fairly easy to reliably cause bsods in tcpip.sys on windows 8. it was something like sending out a ping on a slow connection, and then closing the process before the reply came back.

I don't recall if you had to do that before g finder was added or if it was there all along

of course, real pros didn't just have MacsBug installed, we had The Debugger by Steve Jasik for all of our low-level source-level debugging needs, with its green menu bar and perpetual System 6 (if that) look and feel
