The Great Twist
I had the same problem with FF XIV DX 11, with a GTX 970. It eventually followed me into DX9 and the occasional other game. It turns out the card would poo poo the bed at 70 degrees or so, which I didn't think was dangerously high for a video card. The stupid piece of poo poo also capped its fan speed at 20% which is the reason the temp got that high to begin with.

I started using MSI afterburner to actually let the fan spin. Temperatures dropped dramatically and I've been 100% stable in game for months now.
