I am sure it will be ghosts in the end, thanks to some medical professional getting his own movie. But right now, it IS ambiguous, and Fitz does not look bad because of it.

Artificially induced or untrained astral projection could explain the look of the "ghosts" in the show. After seeing Doctor Strange (which is quite excellent) I'd be happy if they would go that way and it's all the tie-in they need.
