Because insults exist in a wider context, sex organs not dictating gender is true but it's still a quite specifically progressive view that lots of people would disagree with and that certainly hasn't permeated as deeply as 'women are weak and can be battered' and 'vagina = woman', which were clearly the ideas being played off of in the joke.
