Doesn't necessarily exclude the other option though. Not saying it is that, just saying it could be a plausible reason. I've had three doctors refuse treating acute pain even while i was perfectly polite. (Some reaction against a medicine caused my hands to feel itchy to the point of pain, and touching anything, or a breeze of wind, were agony.) In that case they refused simple because they were skin doctors who work mostly not to heal people, but to fleece people of their euros and their bookings were full of patients paying premiums.

Just FYI if you are in acute pain all an Kieferorthopäde can do must of the time is to write you a prescription for an Antibioticum to fight the inflammation. Only once that is gone will he actually do the operation you need.

His classmates also said he said "i'll kill you" so much that they had stopped taking it seriously.

Apparently he had ADHD and it sounds more and more like he was also on the autism spectrum. If he said all his National Socialist beliefs out loud I can totally see his peers reacting like they did.
