Large refugee camps are breeding grounds for really nasty bugs, so combine that with what's festered in NK due to their isolation over the past 60 years and yeah, it's a health hazard and part of the humanitarian crisis a conflict with the DPRK would result in. This is without getting into the real worst case of DPRK biological agents being deployed.
