I am 99% sure that Tourette syndrome doesn't just make you use inappropriate synonyms while talking, does it? And even if it does, does he think the only objectionable thing he did that night was use the word tits? Tourettes isn't going to make you be a loud crass rear end in a top hat about unattractive women in public while talking with your friends who then, when confronted, doubles down by calling her unattractive and asking to see her tits. I do not have tourettes myself but I used to have a friend who did and I definitely got the impression both from him and from medical literature that you swear impulsively as a tic, at random times just like the compulsive shrugging you mention, which is a completely different thing than just using swearing or inappropriate synonyms in a sentence

I have Tourette's. I was diagnosed by a neurologist as a kid. I don't have any verbal tics in the way of words, but I did have one or two with my throat that made weird sounds (thankfully those stopped manifesting much as an adult). Growing up as the kid who made weird movements and sounds he couldn't control well but Totally Wasn't Retarded was a lot of fun.
