Well he's been injured for long periods of time, either "can't play" injured or "carrying an injury so can't play at anything near 100%" which could explain it, but he's just never really looked like he's up for it anymore, or in any way in synch with his teammates. Of course if the rumours about Mourinho not running attacking drills in training are true that could explain it, but I would bet my money on "he's gone physically".

He hasn’t had a full offseason since 2013, can’t imagine that helped.

Sanchez definitely benefits from an attacking system but his entire game is built around being a physical all action attacker. The older you get the harder it is to play that role, especially when as mentioned you have played a lot more games than other pros the same age. Likely early burnout, unless he changes his style he is a money pit now.
