Anecdotal of course:
My friend had a job at the local Lions Club/park doing groundskeeping etc, and when the travelling carnival came to town, as it did every year, he would talk to the carnies, and they all told him "yeah every time we assemble the rides, we have poo poo (nuts and bolts) left over". They also mentioned that it wasn't always the same nuts/bolts.

He took the stairs to the seventh floor and saw blood dripping down the elevator doors.

Shota Tanaka, 31, a passenger from Bethesda, Md., told police he got on an elevator on the fourth floor and pushed the button for the eighth floor. As the elevator moved, he heard "a sound like metal breaking and the elevator shook," the report says. At the eighth floor, the door opened only partially, and blood started to "pour down" between the elevator door and the outside of the elevator, according to the police report.

Amazon held a celebration for the 2 year anniversary of the warehouse that I work in. The celebration happened before and after each shift and it was held outdoors. I have the night shift so I came before ths shift started. It was dark outside and they didn't bother having any lights ready, so people had to walk around and eat in darkness. It was really dumb and unsafe.

Maybe it has desensitized me, but FWIW, I was in the audience for the Defcon 22 elevator talk, and that loving thing gave me nightmares that night (and, among other unrelated things, did not make for a fun flight home). This video is not good, but far from the worst, IMO. Still just additional reasons for me to do more of a leap than a step out of elevators, though.
