Yes, Berman told Ira Steven Behr that Farrell had abruptly quit the show, which is why she gets chumped in "Tears of the Prophets." Behr later said that if he had known about Farrell's request to go part-time, he would have absolutely found a way to accommodate her.

Edit: Behr also gets high on his own farts a lot of the time, though, so I tend to take a lot of what he says with a grain of salt.

What’s sillier is that Cirroc Lofton, despite being a credited star of the show probably was in less than 50% of each season and was in fewer episodes than some recurring cast members. They could have easily accommodated Farrell’s request!

And it only got published because of a happy coincidence. One of the Pocket Books writers who worked on Trek heard Robinson read an excerpt from the journal at a convention, and approached him and suggested that he submit it to Pocket for publication.

Oh, absolutely. At that point, Jadzia had two functions: Worf's love interest, and calling out damage reports on the Defiant. She was a nothing character and she wouldn't have been missed at all in a part-time role.

With all of the incidents with holodecks on starships, how come they are not banned, or serious new safety procedures are not put into place on these things? Does the federation just not believe in safety?

They literally didn't have seatbelts on starships until the Enterprise-E got refitted at the end of Nemesis. And in the era of TUC, they had rolling chairs on the bridge.

Starfleet officers are thrill seekers, that's why they don't wear seatbelts, their computer consoles are made out of rocks and explosives and they still use the holodeck for recreation despite there being a pretty decent chance the holograms will go wrong and murder them.

I remember when I was watching through TNG one moment really stuck with me, when the bridge gets hit at one point and Johnathan Frakes does this unnecessarily elaborate tumble, while in every other scene they just grab the scenery and shake a little. It looked really good and must have required some acrobatic ability to pull off. His legs go all stiff and he just flops over and does a full roll to the side.

Agreed, but isn't it actually historically about Trek aping real-world space agency iconography? NASA, Roscosmos, CSNA, ISRO, and (debatably) JAXA all have some variant of the delta in their logos. The shape just seems to be generally associated with "space stuff" IRL.

Yeah, but CVN-65 was built in 1958. And there were 7 USS Enterprise’s prior to that, going back to 1775.
