Downwash and torqueclamping and the current weight caused the crash but two of the deaths might have come from lack of egress training. On top of all that there were a lot of regulations ignored or shortcut in that 24hrs.

The wires are stripped way too far back. Bare copper like that is a hazard for accidental connections and arcing, and then you have an electrical fire. To me, it also looks like there's too much wire in that junction box, but I'm not an electrician. You don't want to stuff boxes like that full of wires as a general principle, as they could generate too much heat, and again, fire hazard. There are codes and guidelines for these things though.

Looks like half of it got into VRS; it starts to wobble, and then the engine over the deck loses thrust. Report suggests it was downwash bouncing back at the aircraft coupled with possibly being overweight.
