Paternal grandparents were uncle and niece, parents were first cousins. Gross, but not out of the ordinary in some tribal or clan-based cultures, plus at least some new DNA was incorporated at each generation

Looks like the patient was Japanese - there's at least a few coastal cities (towns tbh) where the locals are almost all inbred like that due to the way that they are surrounded by mountains on 3 sides and the sea on 1 side and had very little contact with the rest of the country until very recently. My wife's home city has a population of ~100k and several times the number of mental health hospitals as Yokohama which is, what, 3 million or so people, and her dad (who wasn't born there) likes to joke he's the only man his age he knows who isn't his own uncle.
