When you kill an animal most of the bad stuff you don’t want mixing with the meat you’ll eventually eat is in the stomach and intestines, so it’s important to get that out as soon as possible. If in the process of removing those parts you accidentally puncture something you might end up leaking poop and bacteria on the good bits. Instead of trying to cut the intestines out with a knife this device let’s you do it more cleanly. It’s inserted into the b-hole, twisted so the barbs grab, and pulled out. You can then cut the end of the intestines and tie them off outside the body cavity where any leakage won’t contaminate the meat.

You absolutely do not have to pull the guts put through the butthole to keep the contents off the meat. You don't even have to remove the guts at all to do a thorough job at field dressing an animal.
