It's mostly just poo poo silly dances and skits, interspersed with influencers peddling dangerous products to children. Even with a new account they can still get some data from your email address, IP, and whatever you put in as your age and gender. They absolutely don't care about quality of the content they serve, but some things have to go right (wrong) for you to get served a lot of flat earther videos.

I don’t think it’s possible to explore the concept of a “fresh” TikTok account as a single user. Assuming the idea is that you just let it autoplay forever in full (without giving it the active inputs of flicking to the next video or favouriting), it will take your watching a video in full as an input. Even it served you the first video entirely at random, the algorithm now knows “user watched full video with attributes x, y, z…; adjust algorithm accordingly”.
