The recent show on the Cuban Embargo and Miss America was mind-blowing. I kind of want to know how that show is put together because they're doing much better journalism than even the Daily Show does at its peak. I was really involved with the Broadcast Journalism community at my university, and it just feels like there must be some news director they brought out of retirement with the instructions of, "go nuts." Yeah, the article from a couple of pages ago mentions that a former guy at the New Yorker is head of the fact checking there, thin they got a lady from Washpo as well.

I worked for a PBM. Insurance companies paid us $10 per patient per year for admin overhead and we negotiated bulk purchase rates with pharmacies that every person on the insurance plan can access -unless the insurer specifically writes a rule in the system to tell my adjudicator to refuse to pass on the discount for a given drug in a given plan. Very few of those rules we're written for Viagra and I honestly don't know any other ed drug so I never queried anything else. But Viagra should be the canary in that coal mine as it is the best known. In the statements to customers our discounts(front loaded rebates actually) are typically represented as money provided by the insurer even though it didn't cost the insurance plan anything more than the $10 a year for membership. We also negotiated lower rates for filling RX's from pharmacies so -unless the employer was a dick who demanded we not allow a given drug to be bought on plan - it also made that cheaper for everyone/thing. Whether or not the insurance plan kicked in anything above and beyond the massive discounts (45% off would be the low end ... Some drugs were 90% off when bought by the PBM) varied from drug to drug. And not many did put anything on top for Viagra or BC. But that just makes refusal to cover at all more spiteful because the coverage litterally costs the plan nothing. Furthermore. Our ability to negotiate discounts is contingent on our volume. The more drugs bought on plan the better deals we can negotiate. This is not done drug per drug but manufacturer by manufacturer. Some new drugs might get the individual treatment but the existing catalog would basically be divided into brand and generic and discounts(rebates really but we front load them to look like discounts) negotiated across the boards. So when rear end in a top hat employer says we have to reject drug claims for something, and the manufacturer also makes other things like heart meds, they are raising everyone's price on the heart meds by lowering our total volume for that manufacturer and thus reducing the discount we can get on the heart meds. It literally hurts everyone to refuse to offer at least the basic discount coverage on anything. Which is why it almost never happens except when control freak theocrats put the screws to us and say they won't use our service at all unless we shove their bible up their employees butts for them. Insurers that don't use a PBM are also making everything more expensive for everyone by making the plan/patients pay way more out of pocket for drugs than they should. Insurers smart enough to pay way less for drugs by using a PBM never refuse basic PBM drug coverage on anything unless an employer makes them. Edit: Also there is no possible way we could have refused coverage based on intended use because we have to process a claim in two seconds of the pharmacy submitting it and we literally didn't have that information in the database. According to my training it would have been illegal to collect or store that info for us as we have no need to know it. If they weren't insane, they would also argue that things Men use that Women don't shouldn't be covered either. Really, they're just being dumb about the way insurance pooling works to come up with a "witty" argument. However, the vast majority of posters in this thread range between full healthcare coverage for all to single payer to socialized medicine.
