I don't understand you at all. It's a pretty straightforward comic. The child has an obvious shoulder injury, which is why he is not playing football with the other kids. The neighbor asks how the MOTHER is, because she hates children - that's why she's holding the broom - she's going to chase off the children playing football. The mother says "FINE FINE," because she doesn't want the neighbor to know that she can't get medical attention for her child. The dog dorg holds up his hind leg to reinforce the lame state of the child. Heh. It's pretty funny!

I think it's that the boy was supposed to carry the groceries home, but instead stopped to play with the other kids (hence the obvious injuries on him and the dorg), which in turn made his mom come get him and because the neighbor is watching she can't beat up the boy.
