Both the prostate and a lot of bowel cancers are palpable on DRE, and a lot of bowel cancers metastasize to the spine. So we'll do them on men with urinary complaints and people with unexplained back pain or frank rectal bleeding, plus others. Also for things like sphincter tone in trauma cases. Not super common outside the ED but still done. Endoscopy gets arranged subsequently for a more definitive assessment if indicated.
