Two companions definitely seems to work better as it gives the writers another reoccurring main character to play around with. I was hoping Danny would at least travel with the Doctor a bit, but it wasn't meant to be.

The whole "Arctic Base/Aliens" bit of the dream was from Shona based on the list at the end and the episode hints a lot that the first guy wasn't real (he was an angry douchebag, exactly as Shona felt about him when she fell asleep.) It's actually a surprisingly deep little bit for Moffat in that he resisted hammering the point in.

They really need to poo poo or get off the pot.

Either make a very story heavy continuous arc that spans several episodes or over a season or go back to the barely-mattering stuff like Bad Wolf where it's just a tiny thing in all but the finales (which the last season did mostly do. Shame the finale itself was dire.)

Yeah Nightmare in Silver really is one of those ones that needed a bit of breathing room. You could have done similar to the Orient Express one (though that one was pretty well paced for being a single episode.)
