He's not dying, it's more that he's going to need one at some point in the future. Also, given that it's his half brother, I think there's some preferred child poo poo going on here and I think he is right to look out for himself in this situation. Stepdad seems to be the one pushing this and even though Stepdad has been the only dad this guy has ever known they still don't have a good relationship. The whole thing stinks. Losing your kidney is a big deal, you are basically voluntarily cutting a decade off your lifespan and that's assuming you don't have kidney problems yourself down the line.
