The thing you are thinking about Aetna is that they are now denying hospital visits if the customer was incorrect in their assumption (so if you go to the hospital for extreme stomach pain because you think it's appendicitis, you're on the hook for the $12,000+ bill in its entirety if you're wrong). They're only doing it in five states though (but plan to extend that to the rest of the country in the near future!).

Interesting story about Cigna. They denied me a surgery I needed to help me breathe (no I'm not fat) and they denied it because they said "all known cures for this disorder are considered experimental". Being the complete nutcase that I am, I read their citation and actually went out and got the $400 textbook they cited (OK I bought it on ebay and then sold it back right away but still).

They completely made up the citation. I got the book and it actually said the literal opposite of what they said it said. I brought this to their attention, but it was after my third appeal fell through, so they (intelligently) didn't respond to my finding. I asked them where I should report their medical citation fraud to.

About a year later they completely changed their policy on my breathing disorder and cover it without issue now. Keep in mind that nothing changed; their policy was the same for decades and the same textbook they were still citing was from 2003. So though they hosed me, I like to think I probably helped saved someone else a lot of grief and aggravation (and tens of thousands of dollars).
